A car collided with another Tuesday evening on way to Herat-Kandahar highway.
Ibrahim Mohammadi, director of the Herat ambulance service, said two dead and three wounded were taken to hospital.
A woman was among the dead, he said.
It follows another car crash in the province which took place in the same area.
According to officials, more than a dozen were killed or wounded in the accident.
